These are badges (like good reply, popular link, etc) that have the option to be used as titles. They are only granted to those who get top 3 in official makecode arcade game jams , and cannot be granted from unnoficial ones hosted by users. This is why the winners from stuff like the Unique-a-thon or Coder King don’t have custom titles, as those are unnoficial (like this one.)
